AMP Clinic accepts referrals from physicians and nurse practitioners across Ontario. Physician referrals are required for psychiatric medication consultation and ketamine treatment.

To refer a patient, please download the AMP referral form, complete it in full, and fax it to our secure clinical line. Our team reviews all referrals and will contact you within 2–3 business days of receipt.
